Scope

THS Southern Region

Service provided:

  • Individual assessment, agreed treatment and management plan.
  • Cardiopulmonary rehabilitation: Eight weeks of twice weekly supervised exercise (one hour) and once weekly interactive education with self-management skills training (45 minutes).
  • Supervised exercise sessions (one hour), twice weekly, for those requiring ongoing supervision, especially those in the high risk category.

Referral process

A referral is required for all new appointments and must conform to the Referral Standards as outlined on For Clinicians. eReferral is now the preferred method of referral for this service and is available in all THS regions. Referrals are registered by the clinic to whom they are referred and are triaged according to the specific clinic guidelines.
